Benefit Summary Date of Eligibility
Health Insurance

(Member discounts: massage therapy, fitness club, safety helmets, hearing aids, weight watchers, laser eye surgery, contact lenses and eyewear)
Preferred Blue is a preferred provider organization (PPO) plan. A PPO has a network of participating providers. These health care professionals have agreed to give care and file claims for PPO members.

In a PPO, you decide what doctor you want to see, in or out-of-network. Whether you see an in-network doctor or an out-of-network doctor will affect how much you pay for your health care services.

Through Preferred Blue and Blue Cross and Blue Shield's BlueCard PPO program, you have access to local, national and international network of providers. When you use a doctor or hospital in any of our networks (network care), you'll pay fewer out-of-pocket costs.
Upon hire or during open enrollment. (ioMosaic currently contributes 100% of the cost of the PPO program)
Dental Insurance Designed to allow employees open access to any Delta Dental provider nationwide. Upon employment or during open enrollment (100% currently paid by ioMosaic)
Life Insurance 2x Base Annual Earnings with a $100,000 guarantee issue maximum. Upon employment (100% currently paid by ioMosaic)
Short Term Disability (STD) After 7 days, plan pays up to 60% of first $3,000 of Base Weekly Earnings up to a maximum of 12 weeks Upon employment or during open enrollment (100% currently paid by ioMosaic)
Long Term Disability (LTD) After 90 days, plan pays up to 60% of Base Monthly Earnings up to a maximum of $8,000 per month Upon employment (100% currently paid by ioMosaic)
401k Many funds to select from Contribution may begin the first pay period of the month following date of hire
Vacation for full-time employees (part-time employees pro-rated) 3 weeks per year Accrual begins upon employment Holidays, 7 fixed
Sick Days As needed, upon approval Upon employment
Direct Deposit Deposit of your paycheck in the bank of your choice Upon Employment or anytime thereafter
Flexible Spending Account Up to $4,000/year Upon Employment or anytime thereafter
