Sharing and reporting of near misses and incidents does not always occur, to the detriment of organizational safety or productivity.

In December 2007, a devastating explosion and fire occurred at an R&D agricultural pilot plant at the University of Rio Cuarto (UNRC) in Argentina, resulting in six fatalities, 24 severe burn injuries, and the emergency evacuation of an adjacent daycare center.

The investigation directly linked the incident to deficiencies in safe chemical handling, hazard identification, emergency preparedness, and safety culture. Without a formal system to capture and share historical lessons learned, formal training becomes the essential mechanism to bridge the gap and prevent multi-million-dollar losses.
